HST is a very general progressive overload scheme which I believe sacrifices the true adaptive stress of a particular periodized micro/meso-cycle.
That said, the plan is just that specific for hypertrophy and not so much "recomposition." Usually your diet can accomodate most training protocols, though this leaves much to be desired in my honest estimation regarding "recomposition."
I am not denouncing the program - but it is likely too specialized for what we are trying to accomplish in a CKD.
With the said,...

Most often the answer depends on the days of the week that you train.

2. What about some type of insulin modulation like CHO's on Mon. and Wed. for breakfast and post workout, then no CHO's again until the weekend?

Yes, insulin modulation (rather than glycogen depletion) would be pretty much your only option with the HST (not the DC dependent upon your setup).
I would probably approximate a CKD protocol (if any) for HST to look as follows:
Monday: Low Carb
Tuesday: Moderate Carb
Wednesday: Low Carb
Thursday: Moderate Carb
Friday: Low Carb
Saturday + Sunday: Moderate --> High Carb

Well...you have to come to terms with a desired goal (one that would stand out to you above all else; est and train for attainment of that goal). If the goal centers on hypertrophy...it may be best to not employ the above because a hypERcaloric state is needed in many instances and while hypERcarloric and hypOglycemic are not necessarily entities that must occur in vacuum; they may not be the "best" way.
