I'd say weak cuff over shoulder inflexibility, stretching this out too much could create a bit of laxity (proper term?) in the joint, similar to overstretching a rubber band, flexibility is good, as it seems with age we lose it, and some fall prey to being quarter pressers, but I feel the big problem, is flat bench seems to be the power strength what can you do movement for the upper body, too much focus on how much your moving, rather than what muscles are truly moving the weight.
