i click on the 'instert image' icon and paste the url where the image is located.
if the image is saved on your computer you can upload it to photobucket or something similar to get a web link to use.

Since you are attaching them, one way is preview your post. Select the thumbnail, and it should open up in a new tab/browser. Right click and copy image location. then paste in between the [img] tags...

You can't get rid of the thumbnails when you attach. The best you can do is insert them inline

by using the [attach] tags and in between using the attachment ID...

Oh and the first one is uber hawt. Who is that?
